Parents, Complete Your Athletics Registration Online!

 

There are four steps you must follow to become fully registered for athletic participation.

Parents and Student/Athletes should complete this process together.

Step One: Read the handouts from the WIAA regarding Eligibility and Sportsmanship, the ImPACT Baseline Testing Consent Form (High School only), and the handouts about Concussions and Head Injury Information and Sudden Cardiac Arrest Information. There is a separate concussion handout for athletes and for parents.

Step Two: Complete the Registration Process

Step Three: Submit your signed Physical Card (if needed, see below). 

Step Four:  Athletic Code Agreement Form - We must have a physical copy of the athletic code agreement form signed by the student and parent on file after reading the The Athletic Code Handbook from the Diocese for Middle School or High School participation.  

Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following year.  Athletes WILL NOT be allowed to participate without having a current physical exam on file.

Once you complete these steps, you are registered and can participate in Aquinas Athletics.

***Please be advised….Each sport must be registered individually.  Complete the process for the first sport, then go back in.  Most boxes will prefill with your information.***
