Step 4. Complete Athletics Handbook Agreement form

Once you have read the Aquinas Athletics Handbook, both the student and parent will need to sign the Athletics Agreement Form.  This form must be submitted in hard copy form to the Athletics Office.  The form is valid for either your Middle School (6th-8th grade) years, OR your High School (9th-12th grade) years.  If you are a new student to Aquinas, are participating in athletics for the first time, are an incoming 9th grader, or are an incoming 7th grader (who did not participate in any Aquinas Middle School events as a 6th grader), then you would need to turn in the Aquinas Athletics Agreement Form.

*If you are an incoming 8th, 10th, 11th or 12th grader who participated in athletics during the previous school year, you will already have your athletic code agreement on file and will not need to submit a new form.
