Step 3: Turn in the Physical Form

Step 3: Athletes must have a current sports qualifying physical exam on file in the Athletics Office.  Physical examination taken April 1 and thereafter is valid for the following two school years; physical examination taken before April 1 is valid only for the remainder of that school year and the following school year.  You may either turn in a physical form to the athletics office, or upload your physical form online during the registration process.
